Job Description

• Provides on going discharge planning to facilitate the patient /family caregivers in collaboration with interdisciplinary team to progress toward goals and have a safe timely discharge plan.
• Provides updates to insurances as weekly and as needed to ensure ongoing inpatient approval.
• To be knowledgeable with medicare/Medicaid regulations.
• In consultation with unit professional staff, shall provide supportive counseling to patients and their families, with a focus on helping them cope with traumatic loss.
• Shall elicit from patients and /or their families pertinent information about patients premorbid social, emotional, vocational and spiritual status, and shall communicate this information to the interdisciplinary team in a coherent and relevant manner.
• Shall effectively communicate to the interdisciplinary team any concerns raised by the patient or family
• Shall participate in discharge planning and assist with post-discharge referrals for mental health or substance abuse.
• Utilize knowledge of payer limitations, cost, services and availability of resources to help patients and families make realistic, cost-effective decisions about patient care.
