Job Title: Occupational Therapist (OT)
Location: Skowhegan, ME
We are seeking a skilled and compassionate Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) to join a skilled nursing and rehabilitation team. The therapist will assess, plan, and implement individualized treatment programs aimed at improving patients’ independence, mobility, and ability to perform daily activities.
Key Responsibilities:Evaluate patients’ functional abilities, cognitive levels, and daily living performance.
Develop and execute tailored treatment plans to promote patient independence and recovery.
Provide therapy interventions, adaptive equipment recommendations, and home exercise education.
Document all evaluations, progress notes, and treatment outcomes per facility and regulatory standards.
Educate and support patients, families, and caregivers regarding therapy techniques and home modifications.
Work collaboratively with a multidisciplinary healthcare team to ensure holistic, patient-centered care.
Maintain compliance with safety, infection control, and ethical standards.
Participate in staff meetings, care planning sessions, and quality improvement initiatives.
Active OT or COTA license in the state of Maine (or eligibility to obtain before start).
Graduate of an accredited Occupational Therapy or Occupational Therapy Assistant program.
Minimum 1 year of recent experience in a skilled nursing, rehabilitation, or long-term care environment preferred.
BLS certification (American Heart Association preferred).
Excellent clinical reasoning, communication, and documentation skills.
Ability to work independently and effectively as part of a team.
