Job Description
JOB SUMMARY :
· Responsible for the coordination of clinical activities of the Wound Care Center.
· These include, but are not limited to, utilization of skills in organization, planning, implementing, evaluating, and providing care through the use of hospital and nursing standards.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES :
· Provides case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patient care.
· Provides efficient and effective care with the guidance of the physician during the clinic sessions, attends the clinics where scheduled and is dependable.
· Able to manage a number of patients at the same time with high energy and organization.
· Collaborates with other health care providers, wound care center physicians, Program Director, Medical Director and Supervisor regarding clinic and patient needs.

· Demonstrates thorough, accurate and timely assessments while exhibiting necessary response to changes in the patient's condition.
· Provides and documents care per hospital and department policies and procedures. Demonstrates competence in clinical and technical skills.
· Provides instructions to patient/families regarding cardiology procedures and care following discharge.
· Seeks educational opportunities to improve knowledge/skill.
· Participates in identifying or recommending areas for improvement.
· Completes work in an efficient, timely manner that is conducive to team work and cost containment.
· Communicates in a clear, concise and professional manner.
· Performs nursing care following safety principles, OSHA guidelines and infection control guidelines.
· Provides patient care following the Patient's First Commitment, AIDET, and customer service training.
CERTIFICATION/LICENSE: Cardio-Pulmonary Resuscitation (CPR); Registered Nurse (RN).
ADDITIONAL LICENSURE/CREDENTIAL REQUIREMENTS: Current licensure by the state board of nursing RN. ASN or BSN from an accredited nursing program.
ADDITIONAL EDUCATION AND EXPERIENCE REQUIREMENTS : Mandatory Annual Continuing Education: Customer Service, Fire and Safety, Confidentiality-HIPAA, Infection Control, Corporate Compliance. Annual Unit Specific Competencies, and all education required by regulatory, accreditation bodies and/or Hancock Regional Hospital.
PHYSICAL/MENTAL DEMANDS :
· Lift to 50 lbs. independently and for weights greater than 50 lbs. must use mechanical device or team lifting.
· Push/Pull patients of up to 250 lbs. in a wheeled device and for weights greater than 250 lbs. must use team assistance.
· Install equipment weighing up to 10 lbs. (trapeze, traction) overhead in patient rooms
· Manual dexterity, as used for performing intricate procedures (catheter, dressing changes, IV starts), assembling medical equipment, caring for patients, typing, etc.
· Must be able to lift from ground level up above own head
· Sitting up to one hour of day for computer documentation, written reports
· Squatting and bending at waist to reach under beds, repetitive crouching as needed
· Standing, walking, stooping, kneeling, crouching and crawling as needed more than 2/3 of day
EQUIPMENT USED : Hyperbaric Oxygen Chambers, Pyxis, Sensilase, TCOM.
ENVIRONMENTAL CONDITIONS : Long periods of standing and walking.
