Home Health RN / Public Health Nurse , you'll provide one-on-one care to patients in their homes, helping them feel safe, supported, and empowered in their health journey.

Qualifications: - Current RN license in Iowa
- At least 1 year of nursing experience (home health or community health a plus)
- BLS certification (American Heart Association)
- Valid driver's license and dependable transportation
- Bachelor's degree preferred, or equivalent nursing experience
- Organized, self-motivated, and comfortable working independently
- Strong communication and EMR documentation skills
- Own a personal cell phone
What You Will Do: - Visit patients in their homes to provide skilled nursing care and emotional support
- Develop and manage personalized care plans based on each patient's needs
- Administer medications, monitor progress, and educate patients and families
- Coordinate care with physicians and other providers
- Supervise Home Health Aides and ensure care standards are met
- Support community health initiatives and health promotion events
- Participate in on-call rotation (typically 1-2 weekends/month)
- Document all care using an electronic health record system
