Job Description:

Schedule: PRN (As Needed). Orientation schedule of Mon-Friday Dayshift for first 2 weeks of hire.

Shift : Day Shift 6:45am-7:15pm with every other weekend rotation. 

Location: AH Palm Coast HHA   Hospice 150 Memorial Medical Pkwy, Palm Coast, FL  32164 and AH Fish 1055 Saxon Blvd, Orange City, FL 32763 (Covering both facilities but primarily Palm Coast)

• Other duties as assigned. 
• Maintains accurate and thorough documentation per Medicare guidelines.
• Collaborates with healthcare team members to ensure high-quality patient care.
• Ensures compliance with all local, state, and federal regulations.
• Participates in ongoing professional development and training.

QUALIFICATION REQUIREMENTS:

Associate's of Nursing (Required), Bachelor's of NursingBasic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement, Certified Hospice and Palliative Nurse (CHPN) - EV Accredited Issuing Body, Registered Nurse (RN) - EV Accredited Issuing Body

Pay Range:

$31.53 - $52.24
